CASE STUDY 1

Mr. F, 59 years old , Male , Indian. Visited MediAsia Advance Wound Care & Tissue Repair Centre on 5th April 2015 with chief complaint of Left great toe diabetic foot ulcer for past 1 month. Noted wound malodour, sloughy wound with necrotic patch, seropurulent discharge. Underlying diabetes mellitus for past 10 years under follow up in Kulim General Hospital. He underwent serial wound debridement & desloughing with advance wound dressing and was discharged in August 2015. WE MANAGE TO PREVENT GREAT TOE AMPUTATION WITH TOE SALVAGING SURGERY WHICH ONLY AVAILABLE HERE.

Mr.S , 52 years old, Indian,Male, factory worker. Visited MediAsia Advance Wound Care & Tissue Repair Centre on 1st July 2015 with chief complain of Left great toe diabetic foot ulcer for past 10 days. Noted pus oozing from distal aspect of Left great toe and also crusted slough & callus over media aspect. He underwent serial wound debridement & desloughing with advance wound dressing and was discharged in September 2015.

MRS M, 65 YEARS OLD FEMALE , HOUSE WIFE . Visited MediAsia Advance Wound Care & Tissue Repair Centre early April 2016 with chief complain of Right great toe diabetic foot ulcer and extending to entire foot for the past 14 days. Noted pus oozing from distal aspect of right great toe and discolouration at mid-plantar aspect.

OTHER COMORBIDS – HPT, DYSLIPIDEMIA , IHD, NON SMOKER, NEVER CONSUMED ALCOHOL

NEUROPATHY +++ , Pulses DPA PTA +++ , DOPPLER – ++;

ABPI < 1

GLYCEMIC CONTROL – GOOD

NUTRITION  SCORE – POOR   to  MODERATE

FOOTWEAR – NIL

FAMILY SUPPORT Good

DIAGNOSIS : WAGNER (ULCER ) STAGE 4 , PEDIS ( INFECTION )  GRADE 3

She underwent serial wound debridement & desloughing with advance wound dressing untill completed Healing acheived. Pn.Meriam was discharged in late June 2016. Currently She is taking care of her own Food Stall, at Kuala Muda Kedah.

Mr. R, 55 YEARS OLD INDIAN, Left BELOW KNEE AMPUTATION 2 YEARS AGO NOW ON PROSTHETIC LEFT LIMB.

DM FOR THE PAST 20 YEARS, PRESENTED TO OUR CENTER with over 4 months chronic history of infected Achilles tendon and fasciitis. STARTED TO visit multiple clinics AND AT TIMES HOSPITAL with its ongoing INFECTED LEG BUT YET PROPER AWARENESS OR EDUCATION GIVEN TO PRESERVE THE REMAINING LIMB.

OTHER COMORBIDS – HPT, DYSLIPIDEMIA, IHD WITH CCF,

ACTIVE SMOKER, CONSUMES ALCOHOL REGULAR

DIAGNOSIS: – INFECTED ARCHILIS TENDON, with Fasciitis of surrounding tissue.

He underwent serial wound debridement with advance wound dressing since started treatment since 31st October 2015 until complete Healing was achieved Mid April 2016 .

Mr. R. was discharged in late May 2016.

MR G, 65 YEARS OLD INDIAN, RETIRED HEALTH DEPT OFFICER
ACTIVE SMOKER, EX ALCOHOL CONSUMER, DM FOR OVER 20 YEARS

MR G INITIALLY DEVELOPED LEFT DFU OF THE 5TH TOE & PARTIAL RAYS AMPUTATION WAS DONE AT A PRIVATE CLINIC,  14 DAYS LATER LEFT PLANTAR WOUND GOT INFECTED AND STARTED TO SWELL UP.

ON PRESENTATION TO POLIKLINIK MEDIASIA, COMPLAINT OF PAIN OVER LEFT FOOT, FEVER, LOSS OF APPETITE AND DEPRESSED.

OTHER COMORBIDS – HYPERTENSION, DYSLIPIDEMIA, ISCHEMIC HEART DISEASE

ECG – PREVIOUS ISCHEMIC CHANGES NOTED, Bradycardia 53 bpm

GLYCEMIC CONTROL was ACCEPTABLE. NUTRITION SCORE was MODERATE

No pre diabetic FOOTWEAR, BUT Good FAMILY SUPPORT

DIAGNOSIS: – LEFT DIABETIC FOOT ULCER, WAGNER (ULCER) STAGE 4; PEDIS (INFECTION) GRADE 3.

 

Mr.G underwent extensive debridement & de sloughing through Mid-plantar approach. Subsequent follow ups he went through serial debridement’s with advance wound dressing. Completion achieved in 6 months. Mr. G satisfactorily discharged after 6 months of treatment.

Up to date, he has travelled to Indonesia, India and few other countries. Now He determines his Path.
